Care Coordination

Well-structured care coordination has the potential to provide a more focused, dedicated care management approach that would streamline care, improve health outcomes and reduce healthcare costs. Care coordination efforts for providers of ESRD care have shown promising results. DPC would like care coordination efforts to continue both for the sake of people on dialysis and for American taxpayers.
